Updated FHSAA Form Replaces Old Form in The MVA Matriculation Packet |

Each year the Florida High School Athletic Association revises one or more of its forms. The Consent and Release from Liability Certificate, also known as the EL3 has just been revised. Please access this new form by downloading the matriculation packet which was updated with this new form today, June 23. If you have already sent in your completed forms we appreciate you filling out only the new EL3 form and sending it to the Academy to replace the older form.
